THIS IS A LEGAL DOCUMENT WHICH AFFECTS YOUR LEGAL RIGHTS

I, {name}, have enrolled in the personalized health and fitness program offered through AT Jiu-Jitsu NYC, located at 82-19 Northern Blvd, 2nd Floor, Jackson Heights, NY 11372, operated by Professor Alvaro Tautiva and staff (hereinafter “AT Jiu-Jitsu NYC”).

I recognize that this program may involve strenuous physical activity including, but not limited to, muscle strength and endurance training, cardiovascular conditioning, martial arts, Brazilian Jiu-Jitsu, striking, and other fitness activities. I hereby affirm that I am in good physical condition and do not suffer from any known disability or condition that would prevent or limit my participation in this program. I acknowledge that my participation is voluntary and is not required by AT Jiu-Jitsu NYC.

In consideration of my participation, I, {name}, hereby release and discharge AT Jiu-Jitsu NYC and its owners, instructors, employees, and agents from any and all claims, demands, or causes of action arising out of my voluntary participation. I understand that there are inherent risks involved and that I may sustain injuries, including but not limited to heart attacks, muscle strains, muscle tears, broken bones, shin splints, heat exhaustion, injuries to the knees, back, feet, or other parts of the body, illness, or even death. I knowingly and voluntarily assume all such risks and release AT Jiu-Jitsu NYC from any liability now or in the future.

I further acknowledge and agree that AT Jiu-Jitsu NYC is not responsible for providing refunds for any classes, programs, or services that are canceled for any reason, including but not limited to scheduling changes, unforeseen circumstances, or facility issues. Additionally, in the event that AT Jiu-Jitsu NYC ceases operations or shuts down for any reason, I understand and agree that no refunds will be issued for any unused classes, memberships, or services.

By signing this document, I, {name}, acknowledge that I have read, understood, and agreed to all terms and conditions stated above.

I {name}, hereby authorize AT Jiu-Jitsu NYC to record and photograph the likeness, and appearance (the “Images/Recordings”) of the Model identified above, and to use the Images/Recordings in connection with AT Jiu-Jitsu NYC and any AT Jiu-Jitsu NYC promotional material.

Furthermore, I hereby grant AT Jiu-Jitsu NYC, royalty-free, irrevocable permission to use, reproduce, publish, broadcast and distribute the Images/Recordings, in connection with AT Jiu-Jitsu NYC in any works created by AT Jiu-Jitsu NYC.

I represent and warrant that I am at least 18 years of age and have the full legal capacity to executive this release.

Parent(s) or Guardian(s) (if the person is a minor or lacks capacity in the jurisdiction of residence)

Parent warrants and represents that Parent is the legal guardian of Model, and has the full legal capacity to consent to the Shoot and to execute this release OF ALL RIGHTS IN MODEL’S IMAGES.

I have the right to enter this agreement of the Model Identified above.

By signing this document, I, {name} agree to these terms and conditions. 

This is a liability waiver and release for yourself and your legal successors and assigns. It releases from liability AT JIU JITSU NYC, and its respective past and present agents, affiliates, subsidiaries, officers, directors, promoters, managers, coaches, staff, partners, employees, members, volunteers, assigns, representatives, sponsors, advertisers, venue owners/lessors, attorneys, other attendees, and their legal successors and assigns. By entering you acknowledge that you fully understand and agree that your entrance into this venue may lead to COVID-19 exposure and that contracting COVID-19 may result in severe and permanent damage to your health and/or the health of others, including, but not limited to, death, fever, weight loss, irreversible pulmonary, respiratory and/or neurological system damage, mental or emotional distress, temporary or permanent disability, loss of income, loss of employment, loss of financial or other opportunities, medical expenses, which may or may not be covered by insurance, cleaning expenses, mandatory self-quarantine, and other harms and lost opportunities, whether economic, reputational or otherwise.

This Liability Waiver & Release includes your waiver of any and all claims, known and unknown, arising out of, based upon or relating to your attendance at AT JIU JITSU NYC, including negligence or any possible exposure to, or contracting of COVID-19. It also includes any claims or matters that were, have been, or could in any way have been alleged by you arising out or related to your attendance at AT JIU JITSU NYC. By entering AT JIU JITSU NYC you agree to be bound by this Waiver & Release. 

By signing this document, I, {name} agree to these terms and conditions. 

You must cancel your membership 14 days before your date of charge.  If you wish to cancel your account after the 14 days, it is a $20 fee to do so. Refunds will not be provided for any membership or trial once your payment has gone through. Credit will be offered for your payment instead. Monthly memberships renew automatically on {start_date}. By signing this document, I, {name} agree to these terms and conditions. 

For all Cash Payments, there MUST be a card on file, if you pay more than 5 days after your schedule payment, {start_date} we will charge the card on file a $15 Late Fee or Cancellation Fee. By signing this document, I, {name} agree to these terms and conditions. 

We DO NOT offer makeup classes, therefore if you need to freeze your account, it is responsibility of the member to monitor your own account activity and notify us BEFOREHAND. We allow accounts to be frozen for up to SIX WEEKS. If left frozen longer than the six weeks, your account will be cancelled. To unfreeze your account, it's a $25 fee. 

In order to cancel you must email, call or come in person to confirm you do not want to be charged again for the month. It is the responsibility of the member to monitor your own account activity and membership. 

Keep in mind, if you have an overdue payment, you are UNABLE TO ATTEND CLASSES without updating your payment plan first. We will also charge a $15 late fee for any overdue payment past 5 days and your account will be cancelled. Please keep that in mind for future payments. 

It is also a requirement to BOOK classes. Therefore, to avoid interruption of membership, you must update your payment information before coming into class in order to resume your membership.
